A serious intake covers your conditions and diagnoses, everything you take including supplements, past reactions to medications or needles, whether you are pregnant or breastfeeding, and how you are feeling that day. Someone qualified should read it and ask follow-up questions rather than filing it. You should be able to ask who that person is and what license they hold.

In those cases the correct outcome is that you speak to your own doctor first, even if it costs the clinic the booking. A clinic that has never turned anyone away is not a clinic that screens.
